It's the universal (multi-fit) the bolts can be adjusted to fit the bolt pattern. I had to install an after market wheel spacer due to the offset of my particular wheel on my 03 LX470 so that it mounted flush

Yes. It fits with 1 at the top and 2 toward the bottom.
