How to get my badge in a number I reserved

So a while ago I reserved a number for a badge and planned on joining and becoming a member but since then we have moved twice and I had surgery and we had out Jeep stolen. All in 8 months. Now I bought a new suv and i would like to get my badge in the number I have reserved, who do I contact.
